Position Summary
The Director of Rehabilitation oversees the coordination, quality, and delivery of therapy services within our Home Health division. This role ensures compliance with Medicare regulations, supports and develops therapy staff, and promotes patient-centered care in the home setting.
This position may also serve as the Clinical Manager as defined by Medicare Conditions of Participation.
What You’ll DoLeadership & Operations
- Coordinate and manage therapy services for home health patients
- Review referrals and assign therapists based on patient needs
- Lead, mentor, and support therapy staff
- Assist with hiring, onboarding, and training
- Monitor productivity, utilization, and documentation standards
- Participate in interdisciplinary case conferences
Clinical Oversight & Quality
- Review patient cases, documentation, and care plans
- Guide therapists in goal setting and treatment planning
- Conduct chart audits and drive quality improvement
- Perform supervisory visits in the field as needed
Compliance & Documentation
- Ensure all documentation meets Medicare and regulatory standards
- Maintain patient confidentiality and compliance
- Support policy development and best practices
Communication & Culture
- Model clear, kind, and accountable communication
- Represent Freudenthal’s values in every interaction
- Collaborate with leadership on education and growth initiatives
Role Breakdown
~40% Patient Care / ~60% Leadership & Administrative
Qualifications
- Current license as a Physical Therapist (PT) (preferred for this leadership role)
- Graduate of an accredited therapy program
- Minimum 2 years of management or supervisory experience (home health preferred)
- Current CPR certification
- Valid driver’s license, reliable transportation, and required insurance
- Strong clinical judgment and leadership ability
- Excellent communication and organizational skills
