Mosaic Life Care offers both physical and occupational therapy
services for your patients. These patients can choose from
three convenient locations in the Northland: Mosaic Life Care
at Burlington Creek, Mosaic Life Care at Shoal Creek and
Mosaic Life Care at Platte City.
Mosaic Life Care strives to ensure that every patient visit is a
positive experience involving one-on-one care with our staff of
skilled professionals. We accept all major insurance and handle
all insurance pre-authorization needs prior to the patient’s visit.
Mosaic Life Care is health care, re-imagined. We are dedicated to treating the whole person, not just the disease. We have developed an innovative life-care model that includes traditional health care, as well as a focus on other aspects of our patients’ lifestyles that affect their overall wellness.
