Mountain Park Health Center is a nonprofit community health center. For more than 40 years we have been providing whole-patient care for the entire family. Each year we keep nearly 115,000 of our neighbors in Goodyear, Glendale, Tempe, and Phoenix healthy.

We always put our patients first. As a Patient Centered Medical Home, Mountain Park offers all the care you need in one single place. You’ll have access to an entire Care Team of primary care physicians, physician’s assistants, nurse practitioners, clinical pharmacists, behavioral health consultants and registered dietitians all in one exam room.
Fully Accredited
All Mountain Park clinics are accredited by The Joint Commission, which accredits healthcare organizations across the nation. This accreditation reflects Mountain Park’s commitment to quality, safety and employee education.
To maintain the commission’s Gold Seal of Approval™, Mountain Park demonstrates its commitment to certain performance standards and must undergo an on-site survey by a Joint Commission survey team at least every three years. Learn More
