From its inception, passion and integrity have been at the heart of Take Care Private Duty Home Health Care, founded by Susanne S. Wise, RN, MBA. Opening in Sarasota, Florida in 1995 with four employees, Take Care has successfully grown to more than 500 staff with locations in Sarasota, Bradenton, and Venice. Our services range from providing personal care, whether in the client’s home or facility, to professional transportation to helping clients navigate their health care with personal registered nurse geriatric care managers.

Take Care is a licensed private duty home health care agency in the state of Florida. As a licensed agency, Take Care is subject to unannounced state surveys with the Florida Agency for Health Care Administration. With three offices serving our local community, Take Care provides consistency and highest quality in care. Known for her personal touch and commitment, Susanne continues to embrace healthcare challenges and implement innovative changes to allow older adults to age comfortably and safely in their own residence.
