Physical Therapist -
We live in a beautiful area, surrounded by hills and bodies of water with a view of Mt Shasta that is breathtaking. There are plenty of outdoor activities at our fingertips (for your days off) and a real sense of community both in and outside of the Sky Lakes Campus.
Klamath Falls has 4 distinct seasons and is called the City of Sunshine for a reason - the sun shines even when it is snowing!
We are currently renovating a brand new space as a PT Super Center of sorts. We plan to have PT's, OT's and SLP's all working in the same building where real collaborative work can be accomplished. That is why we need to add more Therapists to the team!
Our Physical Therapists plan and administer medically prescribed physical therapy treatment programs for patients to restore function, relieve pain, and prevent disability following disease, injury, or loss of body parts. All while maintaining confidentiality.

Requirements
Required: Licensed to work in Oregon:
Able to stand, walk, bend, squat, and reach frequently. Possesses physical agility and adequate reaction time to respond
quickly and appropriately to unexpected patient care needs. Must be able to see, hear and speak with or without assistive
devices. Must be able to lift 50 pounds during patient mobility occasionally and 25 pounds or less frequently utilizing
proper body mechanics. Needs ability to communicate effectively through reading, writing, and speaking in person or on
telephone.
ESSENTIAL JOB FUNCTIONS:
Under valid ordering provider orders, provides diagnostic and therapy services with appropriate professional judgment for skilled care.
Participates in the identification and referral of patients through screening, informal evaluations and communication with other persons.
Re-evaluates skilled needs, modifies treatment plans, and prepares discharge summaries as warranted.
Prepares and maintains written and electronic documentation as required for the profession and the department in a timely manner, including billing, evaluation reports, treatment plans and daily progress notes.
MARGINAL JOB FUNCTIONS:
Performs other duties as assigned.
