Join our PACE (Program of All-Inclusive Care for the Elderly) team and help older adults live independently, safely, and with dignity in their communities. As a PACE Physical Therapist, you’ll be part of a close-knit interdisciplinary team dedicated to improving mobility, function, and quality of life for our participants.

What You’ll Do:

• Evaluate and treat participants’ mobility and functional needs, creating customized, goal-driven care plans.
• Provide hands-on therapy focusing on posture, strength, endurance, joint mobility, flexibility, and pain management.
• Educate participants, families, and caregivers to support long-term independence.
• Collaborate with an interdisciplinary team (IDT) to ensure seamless, participant-centered care.
• Recommend adaptive equipment and environmental modifications to enhance safety and function.

Requirements

What We’re Looking For:

Licensed Virginia Physical Therapist

Current CPR certification

Strong communication skills, compassion, and a commitment to exceptional care

Physical Demands: Ability to meet the physical requirements of the role, including lifting, bending, and assisting participants as needed.
