At Continuum of Care Rehabilitation (CCR), you’ll work alongside highly qualified strength & conditioning coaches who help progress patients into advanced performance and returntosport programming. We’re a small, tightknit clinic that protects your time, supports your growth, and is actively reducing documentation burden so you can focus on patient care.

What You’ll Do:
Perform orthopedic and musculoskeletal evaluations
Create individualized treatment plans using evidencebased interventions
Provide manual therapy, therapeutic exercise, and functional training
Collaborate with S&C coaches for advanced progression
Track progress and adjust plans to meet patient goals
Educate patients on home programs and longterm wellness
Complete documentation (with systems in place to streamline admin work)
Provide onsite medical coverage for community sports events and local athletic activities.
Represent CCR in community outreach, including participation in social media content, educational posts, and brandbuilding initiatives.

Requirements
Bachelor’s, Master’s, or Doctorate in Physical Therapy
Active Colorado PT license
Strong communication and patientrelationship skills
Interest in orthopedics, sports rehab, or performance training
New grads welcome (if applicable)
